Who’s at the Door? Hallucination & Anomalies Guide

Hey everyone, and welcome to my deep dive into the mind-bending world of Who’s at the door?. If you’ve ever found yourself questioning your own sanity in that creepy apartment, you’re in the right place. I’m breaking down every hallucination I’ve run into, room by room, to help you figure out what’s real and what’s… not.

Developed using Matasol29’s original guide.

List of All Anomalies

So, full disclosure, this guide is a living document. I’ve managed to pin down and document 60 of these spooky events, but the RNG gods are being a bit stingy with the last few. I’m still on the hunt for four more, not even counting the super-secret hidden ones.

The ‘Safe’ Room… Or Is It?

You’d think the Safe Room would be, you know, safe. But sometimes, the game messes with you right from the get-go. One hallucination hits you the moment the day begins.

Bloody handprints on the safe room wall, a startling hallucination.

Another nasty surprise triggers the second you open the door. Nothing like a good morning jump scare, right? You should also keep a close eye on your notepad. Occasionally, it’ll give you a heads-up with some creepy red text, warning you that things are about to get weird.

The notepad shows red text, warning of an upcoming hallucination.

For example, you might see a warning for ‘Lanky Bori’…

Close-up of the notepad warning about Lanky Bori in red text.

…or other fun friends like ‘Lanky Takgu’ in the Living Area and ‘Giant Gomu’ in the Bathroom.

The notepad has a red text warning for Lanky Takgu.

A couple of others to watch for are ‘Crawling Bori’ in the Entryway and the ‘Ghost’ in the Living Area. The sneaky thing about these is the text only turns red *after* you’ve opened the main door.

The Ghost waits for you in the living area right after you open the safe room door.

If you get this unwelcome guest, she’ll be waiting for you in the Living Area. My advice? Quickly shut the door and wait for her to leave. Only then is it safe to venture out.

A close-up of Bori during her story-related hallucination event.

To learn Bori’s story, you have to leave the apartment. But get ready to run, because she will chase you the second her voice lines are over.

A cutscene revealing lore about the character Takgu.

It’s a similar deal for Takgu’s story, which you’ll see in a cutscene outside. I’ve personally never had this one pop up during Endless Mode, so it might be exclusive to the main story.

Surprises in the Living Area

A ghost that only moves when you're not looking at it stands in the living room.

This guy is a real pain. He starts in the Entryway but pulls a Weeping Angel on you, only moving when you’re not looking. You can usually hear his footsteps getting closer, and sometimes you won’t spot him until you’re leaving the Kitchen. Thankfully, he can’t follow you in there. Just don’t let him touch you, unless you’re a fan of jump scares.

A ghostly woman is sitting on the couch and crying.

You’ll know this one is coming. As soon as you leave the safe room, you can hear a woman crying.

A baby monitor on the floor showing static.
A scary face appears on the baby monitor screen.

This creepy baby monitor will be on the floor, and you can hear the static as you get closer. Then there’s the doll, which loves to play hide-and-seek. It first shows up between the couch and the safe room.

A creepy doll appears on the floor next to the couch.

If you look away, it might pop up next to the vase across the room. It can disappear entirely, so it’s always a good idea to double-check after you’ve been in the Entryway.

The creepy doll has moved and is now sitting next to a vase.
An empty space where a ghost was previously, indicating it has disappeared.
The TV screen shows Bori walking down the apartment hallway.
Bori lunges at the screen from the TV.

This one is a classic horror movie trope. The TV will show Bori walking down the hallway towards your door. Then, she’ll jump straight through the screen and chase you until you manage to take the pills.

Sometimes you’ll hear a baby crawling around as soon as you leave the Safe Room.

A shadowy baby figure crawling on the floor in the light.
The spot where the crawling baby was is now empty.

Like some of the other spooks, this one can vanish, so it’s smart to check again after you’ve been to the Entryway.

A ghostly girl stands silently by the bookcase.
The notepad in-game with a red text warning about Lanky Bori.

Remember those notepad warnings? This is another one they can point to.

The notepad warns about 'Lanky Bori' being in the Living Area.
Mori's doll sits on a box in front of a microphone.
View of Mori's doll is obstructed by a plant.
Mori's doll has moved and is now peeking out from behind the microphone.

Mori’s doll is another tricky one. It’ll appear behind the microphone if you look away or if your view gets blocked for a moment.

A notebook lies open on a table in the living area.
The phone on the wall is ringing, indicated by visual effects.

After you check out the notebook, the phone will start ringing. If you answer it, you’ll get some extra bits of lore.

A possessed lamp has moved to the top of a cabinet in the living area.

This one’s a multi-stage nightmare. It starts with a head under the Kitchen sink. If you don’t take your pills fast enough, it’ll move to this cabinet in the Living Area.

The possessed lamp has moved again, now sitting on top of the bookcase.

If you still haven’t dealt with it, the lamp will then teleport to the bookcase once you look away.

The lamp flies through the air towards the player.

You have to be quick with the pills here. If you wait too long, you’ll get a message that says, “I’ve followed you. Look behind you,” and the lamp will fly right at you. Fun fact: if you manage to dodge the lamp, it can sometimes glitch the game and force you to restart.

A bloody newspaper clipping on the floor.

Once you pick up this bloody clipping, the hallucination ends.

Bloody handprints on the wall that can disappear as a hallucination.

These handprints can disappear, so be sure to double-check the walls after walking around.

Kitchen Nightmares

Shards of broken glass suddenly appear in the kitchen sink.

You’ll hear the sound of glass shattering before you even see the shards in the sink.

Opening the fridge reveals a severed pig's head inside.

A good tell for this one is an extra dish appearing by the sink. If you open the fridge, you’ll find a lovely pig’s head inside. You can even hear it oinking if you get close.

A severed head is visible under the kitchen sink.
A head under the kitchen sink, the start of the moving lamp hallucination.

Here’s the start of that moving lamp event I mentioned. If you find the head here and don’t take your pills, it moves to the cabinet in the Living Area.

The lamp has moved to a cabinet in the living room.
The chicken magnet on the fridge is moved, indicating an anomaly.

Sometimes you can spot this one without even opening the cabinet because the chicken magnet on the fridge will be out of place.

The handle of the fridge is covered in blood.

For this one, you’ll hear a baby crying before you even open the fridge. If the head inside gets you, you’re treated to a jump scare from the grey baby doll. Afterwards, the blood and head vanish, and you’re safe to open the front door. Or, you can just take the pills after spotting the blood to clear it.

The freezer compartment handle is covered in blood.

Same deal here. You can take the pills as soon as you see the blood to count it as an anomaly, or you can open the icebox, trigger the jump scare, and have the blood disappear so you can get back to business.

A swarm of flies buzzes around the kitchen sink area.

Your ears are your best friend here—you can hear the flies buzzing if you get close enough.

A ghostly face appears inside the microwave.
A close-up of the creepy face inside the microwave.

Another audio cue! You’ll hear laughter as you approach the microwave.

The kitchen cabinets are shaking as if someone is knocking from inside.

You’ll hear knocking as soon as you step into the Living Area, but it’s coming from the kitchen cabinets.

A small, creepy doll stands on the kitchen windowsill.

For this little guy, you can either take the pills right away to clear the anomaly, or if you get closer, the doll will just vanish, and you can open your door.

Bathroom Scares

The massive figure of Giant Gomu fills the small bathroom.

This is another one of those hallucinations your handy notepad might warn you about.

The notepad shows a red text warning about Giant Gomu in the bathroom.
The cabinet above the toilet is completely empty, a sign of an anomaly.

As an extra clue for this event, the cabinet over the toilet will be suspiciously empty.

The bathtub is filled with blood.
A toothbrush holder is floating in mid-air in the bathroom.
A grey baby doll lunges at the screen in a jumpscare.

This event is a double-whammy. The toothbrush holder will fly at you, and then the grey baby doll will jump you. After the scare, everything goes back to normal, and you’re free to open the front door.

The bathroom mirror shatters and a baby head is thrown from it.

As soon as you walk into the bathroom for this one, the mirror shatters and throws a baby head at you. Charming.

A ghostly face flashes in the bathroom mirror as the lights flicker.

You’ll know this is happening when the lights start to flicker and a face keeps flashing in the mirror.

A baby doll head is sitting in the toilet bowl.
A close up of the baby doll head in the toilet.

Another sign for this hallucination is that the papers in the Entryway will change into missing person posters.

The papers in the entryway have turned into missing posters.

This one requires some legwork. You have to go up to the doll head, then turn around and leave the apartment to find another head in the hallway. Once you pick it up, be ready to sprint back inside, because Takgu will chase you. The door will slam shut behind you once you’re safe.

Takgu chases the player down the hallway.

Entryway Encounters

An old CRT television chases the player down the entryway.

Yes, that’s a haunted, chasing television. You’ll hear it scraping along the floor as it comes after you. Quick tip: the Bathroom door won’t open while the TV is on your tail.

A ghost stands in the entryway, blocking the path.
A side view of the ghost in the entryway.

This one’s pretty chill, all things considered. She just stands there and lets you take the pills to make her go away.

The peephole on the front door is dripping with blood.

This lovely sight only appears after you’ve already looked through the peephole once.

Bori is crawling on the floor of the entryway.

Another potential notepad warning! This time, it’s for ‘Crawling Bori’.

The notepad in-game with a red text warning about Crawling Bori.
A trail of bloody footprints appears on the floor.
The trail of bloody footprints leads from the entryway towards the safe room.

If you see these bloody footprints, don’t follow them! Or do, I’m not your boss. But they lead right back to the Safe Room where a doll is waiting for you.

A doll waits inside the safe room at the end of the footprint trail.

This is also where you can find the missing posters that are a clue for the ‘Baby Head’ event in the bathroom.

Ghostly hands are reaching out from under the front door.

Through the Peephole

Looking through the peephole reveals a giant, unblinking eye staring back.
A close-up of the terrifying eye seen through the peephole.

These peephole-specific hallucinations can happen no matter who is actually knocking at your door.

Looking through the peephole shows a body hanging in the hallway.
A scary face suddenly appears, filling the entire peephole view.
Ms. Gomu is standing silently in the hallway, not knocking.

Normally, each character takes breaks from knocking. The real anomaly is when they just stand there, silent and staring. In my experience, this happens most often with Ms. Gomu.

A man stands silently in the hallway, staring at the door.

The Medical Card

This strange medical card can be found in various spots all around the apartment.
